Summer Camp Medical Forms

Please make sure that your child's medical and dental exams have taken place within a year of their time at camp with us (July 27th, 2012 or AFTER). Exams from Spring 2012 will not satisfy the regulations even if the doctor dates the form more recently. MEDICAL FORMS: Due by MAY 1, 2013 (please contact me if your child has an annual physical scheduled for May or June).

Some of the required forms are different for campers who will be under five years old during any portion of their time at camp. Please be sure to download the correct forms for your child's age.

Please note: Since Lowell is in the District of Columbia, only the DC Certificate is acceptable. We cannot accept school health forms from neighboring jurisdictions as a substitute. Also, all campers must comply with DC regulations regarding required immunizations and PPD and lead screenings. Children who do not live in the District may not have had some of these as part of their routine medical care. Please print out the attached flyer, Immunization Requirements 2011-2012, and review it with your child's pediatrician to determine if all vaccines and screenings have been given.

FORMS FOR CAMPERS WHO WILL BE FIVE (5) OR YOUNGER ON JUNE 18TH 2013: This will be all Pre Primary and Kindergarten Club campers and possibly a few Best Buddies.

DC Universal Health Certificate, Oral Health Assessment Form, Authorization for Child's Emergency Medical Treatment, Registration Record for Child Receiving Care Away From Home. These required forms are found together under the Related Files heading in the link titled Pre Primary & K Club Required Forms Packet.

FORMS FOR CAMPERS AGED SIX (6) AND OLDER (as of June 18, 2013) -

DC Universal Health Certificate: Please print this form from the attachments (upper right hand corner under the Related Files heading).

Forms for ALL AGES:

Authorization to Administer Medication: If your child will or MAY (due to asthma or allergies) need medication administered by our nurse during the camp day, we need a medication authorization form signed by both you and your child's doctor. There is one form for children under five (Pre Primary Medication Authorization Form) and a different form for those five and older (Medicine Authorization Form).

Over the Counter Medication (OTC) Authorization Form: Not required but we will need this if you would like the nurse to give over the counter medicine if needed. Please print the form and have it signed by the health care provider and a parent or guardian.

FORMS FOR KIDS WITH FOOD ALLERGIES OR ASTHMA:

Asthma Action Plan: Please submit this if your child has asthma.

Food Allergy Action Plan: This form, along with a prescription medication authorization form, must be completed for students requiring EpiPen and/or Benadryl for severe food or insect allergies.
